Our pelvic floor is a complex web made up of numerous muscles like levator ani, sacrococcygeus, pubococcygeus, ischiococcygeus, various ligaments, capsules and bones. The entire web of muscles, ligaments and capsules mainly helps to protect and support the delicate pelvic organs like rectum, vagina, bladder, urethra, uterus etc.

During the process of childbirth, the ligaments get stretched and relaxed due to the release of various hormones like relaxin and many more. Therefore, the ligaments get stretched more in a woman who has delivered a baby previously and more so in a multiparous woman. In this process, the pelvic floor muscles become weak due to the trauma caused by childbirth.

Weakening of the pelvic floor aggravates various other problems like urinary incontinence, prolapse/ Descent of pelvic organs, like, bladder, rectum, uterus. The commonest of which is Uterine Prolapse.

It's important for us to understand the importance of pelvic floor muscles and for that Peggy Norton's theory beautifully explains the importance of pelvic floor muscles.

This theory of boat in dry dock, says that let us imagine a boat which is standing at a dock and the level of the water is quite low. In such a situation, as the water level is low, the boat will hang onto the rope and after some time, the rope will get stressed and elongated. A state would come where it would be elongated so much that It breaks, causing the boat to deattach and sink into the water.

If the rope is reattached without increasing the level of the water, the rope with time will again break. Why? Because the cause which is low water level (that caused the boat to sink) has still not been dealt with. So now in our body also, if the pelvic floor muscles are weak, the ligaments will get stressed and elongated and at a certain point it will cross the threshold of elongation and it will breaks, causing a prolapse occurs.

In the presence of Pelvic floor weakness, if an organ suspension surgery is performed, it is possible that a re- prolapse might occur after some time, as the cause of weak pelvic floor is still untreated. For the same reasons, pelvic floor muscle training is important and valuable to prevent incontinence, prolapse and other pelvic pain problems.
