Why scientists thought that BCG vaccine may confer immunity to coronavirus?
1. The lower than expected number of COVID-19 cases in countries with fragile health system is puzzling. Although countries like Nepal, India, Philippines, Sri Lanka, Ccambodia, Thailand and Vietnam have reported their first confirmed case of SARSCoV2 in January, widespread examples of community spread have not been reported. Contrary to such expectations / predictions, Europe that has best healthy system was declared epicenter of pandemic by WHO on 13, arch, 2020.
2. Several vaccines including BCG vaccination have shown to produce the "heterogenous" or non specific immune effects leading to improved response against other non mycobacterial pathogens eg, acute respiratory infections. This is known as trained immunity or off target effects. Scientists believe that this effect may provide immunity to COVID-19.

-- This study was entitled "Correlation between universal BCG vaccination policy and reduced morbidity and mortality for COVID-19 - an epidemiological study." This was conducted by Miller A. et al in New York, U.S.A.

What were the results?
1.Most of the countries with low income levels reportedd zero deaths attributed to COVID-19 and have universal BCG policies in place consistent with a protective role of BCG vaccination.
2. Middle high and high income countries that have a current BCG policy (55 countries) had 0.78 deaths / million people. In contrast, middle high and high income countries that never had a universal BCG policy (5 countries) had a larger mortality rate with 16.39 deaths/million people. This difference between countries was highly significant.
3. Positive significant correlation between the year of establishment of universal BCG vaccination and mortality rate, consistent with the idea that the earlier that a policy was established, a larger fraction of the elderly population would be protected.

Why did COVID-19 spread in China despite having a universal BCG vaccination policy since 1950?
-- During cultural revolution i.e. from 1966 to 1977, tuberculosis prevention and treatment agencies were disbaned and weakened. This could have created a pool of potential hosts that would be affected by and spread of COVID-19.

What is the authors' recommendations?
-- The correlation between the beginning of universal BCG vaccination and the protection against COVID-19 suggests that BCG might confer long-lasting protection against the current strain of coronavirus. However, randomized controlled trials using BCG are required to determine how fast an immune response develops that protects against COVID-19. BCG is generally innocuous with the main side effect the development of inflammation at the site of injection. However, BCG is contraindicated in immune compromised people as well as pregnant women, so care should be taken when applying these possible intervention for COVID-19.
