In New York City, John Wick is getting ready to seek revenge against the High Table while hiding underground with the Bowery King. He travels to Morocco and kills the Elder, the person in charge. As a response, High Table member Marquis Vincent Bisset de Gramont summons the manager and concierge of the New York Continental hotel, Winston and Charon. Marquis explains that the High Table has given him unlimited resources to kill John, and he criticizes Winston for failing before. As punishment, Marquis removes Winston from his managerial duties, declares him "excommunicado," destroys the New York Continental, and executes Charon. Marquis then forces Caine, a retired High Table assassin, to kill his old friend John by threatening to harm Caine's daughter.

John seeks refuge at the Osaka Continental, managed by his friend Shimazu Koji. Marquis' second-in-command Chidi, along with High Table assassins and Caine, arrives to investigate the hotel. Koji's daughter Akira, the hotel's concierge, evacuates everyone just before the High Table "deconsecrates" it, causing chaos. John fights through waves of armored assassins, leading to a showdown against Caine. A bounty hunter named Mr. Nobody interrupts their fight and allows John to escape after finding the current bounty insufficient. Koji, despite being wounded, helps John by attacking Caine until Caine reluctantly kills Koji but spares Akira.

John returns to New York and meets Winston at Charon's gravesite. Winston suggests a High Table tradition to challenge Marquis to a duel, with the winner being free from High Table obligations. John must request a duel on behalf of a crime family, so he goes to the Berlin headquarters of the Ruska Roma crime syndicate, requesting readmission by killing Killa Harkan, a High Table member who murdered Katia's father. John succeeds with the help of Caine and Mr. Nobody, regaining his status. Winston relays John's challenge to the Marquis, acting as John's 'second,' and proposes terms.

In Paris, John and the Marquis decide on the duel's parameters in a meeting moderated by the Harbinger. The Marquis nominates Caine to take his place, and the Harbinger warns of consequences if either party fails to appear. The Bowery King arrives in Paris, providing John with a pistol and a new ballistic suit. The Marquis places a bounty on John to prevent him from reaching the duel. John fights off numerous assassins, including Mr. Nobody, and arrives just in time. The duel takes place at Sacré-Cœur, and after intense rounds, John executes the Marquis.

The Harbinger grants Caine and John freedom from the High Table, reinstates Winston as Hotel Manager, and pledges to rebuild the New York Continental. John reflects on his life and marriage before succumbing to his injuries. In a post-credits scene, Caine returns to Paris but is approached by a vengeful Akira.
