This includes Godzilla DLC Monsters despite only having recolors as well as Onslaught Skins.
Notes:
Skins 1-4 are available by default except Kongkrete who has 6 Skins only.
Skins 5-9 (2-6 for Kongkrete) are unlocked by leveling up each Titan/Kaiju.
Skins 10-12 are unlocked by completing specific waves in Onslaught Mode, highest is 50 and you get these skins once you beat the wave that's equipping these Skins.
Lores are also unlocked by leveling up each Titan/Kaiju with the exception of the DLC Kaijus who have descriptions which are lengthy.
